Intake / 1st Visit
On the first visit you will meet with your intake clinician to complete paperwork, and discuss the reason you are seeking services. The intake clinician will explain our services, and together you will create a plan of care.
If the client is a child (under 18), he or she must be present with the parent or guardian for the intake process.
Upon arrival sign in at the reception desk, and have ready to present the following:
- Photo ID
- Soonercare/Medicaid or Medicare Card
- Private Ins. Card (e.g. BCBS, Health Choice, etc.)
- Proof of Income (at least one month of check stubs)
